Most people say they would choose end-of-life care in the comfort of their own home, surrounded by friends and family. We honor that choice and help make it happen.

Hospice care can be initiated in any home setting including a nursing home, adult family home, group home, or assisted living facility. Wherever you are at home—we'll be there.

We Can Help

Hospice care is appropriate for patients living with a condition that is not considered curable, and has progressed to where there may be six months or less of life.

Patients of any age and with any terminal illness are eligible.

This includes illnesses such as:

  • Dementia
  • Heart Disease
  • Liver Disease
  • Cancer
  • AIDS
  • COPD/Lung Disease
  • Neurological Diseases
  • Kidney Disease

Hospice addresses all the needs of the patient and family during the last stages of an illness, not just the last hours, days, or weeks of life. Patients who contact us when they first learn of an illness is incurable are able to receive the most comprehensive care.

Hospice Services

Hospice provides case management by a team trained in end-of-life care.

  • Coordination of care with patient's primary physician
  • Regularly scheduled RN visits
  • On-call RN nights and weekends
  • Hospice Aide assistance
  • Social work support
  • Spiritual support
  • Volunteer support
  • Bereavement support
  • Respite care
  • Loan of durable medical equipment
  • Provision of personal care supplies
  • Provision of medications relating to the terminal prognosis
